They are intended to celebrate progress on implementation projects.\u00a0 There are three levels of awards for completion of the checklists for each level of implementation &#8211; Path Finder Level 1, Level 2, and Level 3.<\/p>\n<p>The <strong>Trailblazer<\/strong> award is given in recognition for having implemented guidelines or having processes that have strong fidelity with Bree reports.<\/p>\n<p>The <strong>Mountain Climber<\/strong> award is given for exemplary implementation efforts that focus on the Bree pillars of transformation, i.e. equity, data exchange and transparency, person-centered care, cost\/financing or ROI. Each year will focus on one of these pillars. Fidelity is determined by filling out the &#8220;score card&#8221;, available in the link on <em>Submission Portal<\/em> tab below. Anyone submitting self-report data to the Bree Collaborative who receives an average implementation score of <em>2.00<\/em> or greater for a specific report will automatically receive an award. We inform each submitter by email.<\/p>\n<p>Recipients of the Trailblazer Award will receive:<\/p>\n<ul>\n<li>Award Certificate to display<\/li>\n<li>Recognition on the Bree Collaborative website and in blog posts<\/li>\n<li>Recognition at our annual conferences<\/li>\n<li>Award logos to display on your website, social media, and other communications to advertise your work on best practices to patients and health care partners<\/li>\n<li>Invitation to apply for our highest award &#8211; the Mountain Climber Award<\/li>\n<\/ul>\n<p><em>Trail Blazer Award criteria<\/em><\/p>\n<ul>\n<li>Submission of self-report form, supporting documentation, and average score of 2.00 for fidelity with Bree Guidelines.<\/li>\n<\/ul>\n<p>The format is an excel workbook which contains sheets for each guideline. To complete reporting, choose the relevant report topic (for example, LGBTQ care, Colorectal Cancer Screening, etc.) and fill out the items and metrics for each instance of implementation your organization. Each instance can mean by department, by delivery site or for the entire organization if you are using the reports to inform care throughout your organization.<\/p>\n<p>[\/vc_column_text][vc_btn title=&#8221;More Information about the Trailblazer Award&#8221;
