“Implementation in this second sense involves the programs and activities that take guidelines out of the rather abstract phase of development and into the actual world of health care decision making and action.” 1<\/em><\/p>\n

For the purposes of awards for self-reporting your implementation project the term “implementation” can be understood to mean; use of a guideline in part or full in the course of clinical practice, health care contracting, policy making, educational programs, or other health care related activities; and\/or use of guidelines to fulfill the elements of an initiative, regulation, or requirements.<\/p>\n

The aim of these awards is to highlight and celebrate quality work being done by many different organizations.<\/p>\n

Being Considered for Awards<\/strong><\/p>\n

Any organization can be considered for an award.<\/p>\n

Types of awards<\/strong><\/p>\n

The Pathfinder<\/strong> awards are given automatically. They are intended to celebrate progress on implementation projects.\u00a0 There are three levels of awards for completion of the checklists for each level of implementation – Path Finder Level 1, Level 2, and Level 3.<\/p>\n

The Trailblazer<\/strong> award is given in recognition for having implemented guidelines or having processes that have strong fidelity with Bree reports.<\/p>\n

The Mountain Climber<\/strong> award is given for exemplary implementation efforts that focus on the Bree pillars of transformation, i.e. equity, data exchange and transparency, person-centered care, cost\/financing or ROI. Each year will focus on one of these pillars. Multiple awards are given out annually and any organization receiving a Trail Blazer award is automatically eligible to submit a form to be considered for the Mountain Climber award.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=”Pathfinder Awards” tab_id=”1703181412813-4e77a31e-6682″][vc_column_text css=””]The Pathfinder awards are based on the submission of the Bree Collaborative checklists for each report and supporting documentation or completion of a score card demonstrating basic concordance with Bree recommendations.<\/p>\n
