The Bree Collaborative applauds clinics, hospitals, systems, health plans and other care providers that prioritize the elimination of structural and process barriers to diversity, equity, and inclusion.<\/p>\n<p>The Pathfinder award is given to those organizations that are working to improve their policies, procedures, or other recommendations from our guidelines in any topic area.<\/p>\n<p>Any data or information submitted with this award application is used to populate dashboards and reports on the uptake and use of the Bree guidelines. Unless you are otherwise notified, all use of the data for reports and dashboard is de-identified. Names of award winners will be published on our map and will consist of organization name, locations (s), name of award, date award was given, and type of organizations (school, health system, health plan, etc.)<\/p>\n<p>Any organization that is identified as an &#8220;audience&#8221; (clinician, health system, health plan, etc.) can submit a <a href=\"https:\/\/www.qualityhealth.org\/bree\/checklist-tools\/\">checklist<\/a> or a score card as attestation of implementation of the Bree Collaborative guidelines for a <strong>Pathfinder Award<\/strong>.<\/p>\n<p>Recipients of the Pathfinder awards are acknowledged with:<\/p>\n<ul>\n<li>pins for staff directly involved in the implementation of the guidelines to celebrate their role as champions<\/li>\n<li>award logos to display on your website, social media, and other communications to advertise your work on best practices to patients and health care partners<\/li>\n<\/ul>\n<p><em>Pathfinder Award criteria<\/em><\/p>\n<ul>\n<li>Submission of proof of completion of level 1, 2, or 3 checklist<\/li>\n<li>Submission of supporting documentation<\/li>\n<li>OR submission of an audience specific score card<\/li>\n<\/ul>\n<p>[\/vc_column_text][vc_tta_section title=&#8221;Candidate Submission Process&#8221; tab_id=&#8221;1719934783775-cb000726-72a7&#8243;][vc_column_text css=&#8221;&#8221;]The Pathfinder awards are based on the submission of the Bree Collaborative checklists for each report and supporting documentation. There are three levels of awards for completion of the checklists for each level of implementation &#8211; Path Finder Level 1, Level 2, and Level 3.<\/p>\n<p>Checklists can be found in the Bree Collaborative <a href=\"https:\/\/www.qualityhealth.org\/bree\/implementation-guide-home-page\/ig-topics\/\">Implementation Guide<\/a> under the Implementation Checklists by Audience Tabs or under the implementation tab on the <a href=\"https:\/\/www.qualityhealth.org\/bree\/checklist-tools\/\">checklist<\/a> page.<\/p>\n<p>Checklist submission can be done via email to bree@qualityhealth.org or via the <em>Submission Portal (link)<\/em>[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;Key Dates&#8221; tab_id=&#8221;1719935124212-8ac52e79-adba&#8221;][vc_column_text]<\/p>\n<ul>\n<li>Application open: year round<\/li>\n<li>Award Notifications: year round, on review of submission<\/li>\n<\/ul>\n<p>[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;FAQs&#8221; tab_id=&#8221;1719934784938-6466169a-f346&#8243;][vc_toggle title=&#8221;What are the purpose of checklists?&#8221;]Each report has three levels of checklists, starting with internal activities and increasing towards activities that require collaboration outside of an organization or increased capacity. The purpose of these checklists is to provide structure to implementation projects and allow organizations to move towards full implementation in a systematic and rational way.[\/vc_toggle][vc_toggle title=&#8221;What kind of support documentation should be included?&#8221;]Supporting documentation may include documentation of any of the resources you used during your implementation process. Some examples are worksheets or work books, self-assessments, or other tools used during the implementation process.[\/vc_toggle][vc_toggle title=&#8221;Who should be involved in filling out the checklists?&#8221; css=&#8221;&#8221;]Depending on the report topics you may consider convening a team to check off specific items on the <a href=\"https:\/\/www.qualityhealth.org\/bree\/checklist-tools\/\">checklists<\/a>.
