\r\n\r\nRead our Social Need Interventions Recommendations<\/a><\/span>\r\n\r\nSocial need interventions refer to the many ways that an organization follows-up on identified social need. This includes individual care coordination programs, community information exchanges, partnerships between healthcare and social service organizations, or policy and funding mechanisms that allow for sustainable social services in Washington state. This workgroup works to identify best practices for social need interventions, including staffing and technology needs, innovative partnerships, and existing resources to integrate social care in healthcare.\r\n\r\n\r\n
