The Foundation for Health Care Quality is a nonprofit organization dedicated to providing a trusted, independent, third party resource to all participants in the health care community – including patients, providers, payers, employers, government agencies, and public health professionals.

The Foundation for Health Care Quality is the sponsor for, and the home of, a number of programs which deal with variability, outcomes and quality in various medical and surgical services. In addition, we have been a long time participant in the health care technology arena, and sponsor a major statewide patient safety coalition. Our most recent sponsored program is HealthPact, which focuses on how systems and individuals deal with unanticipated events. Each of these programs has its own portion of this website.

Cardiac
CARE OUTCOMES ASSESSMENT PROGRAM (COAP)

The Clinical Outcomes Assessment Program (COAP) is a regional quality collaborative that leverages medical leadership and clinical, administrative, and financial data to establish and drive best practices in cardiac care. Our purpose is to support all hospitals and clinicians in achieving the highest levels of patient care and outcomes.

CARE OUTCOMES ASSESSMENT PROGRAM (COAP)

Spine COAP is a surgeon-led collaborative that uses performance surveillance and benchmarking to help deliver more appropriate, safer and higher quality surgical spine care across Washington State.

CARE OUTCOMES  ASSESTMENTS PROGRAMS (SCOAP)

The Surgical Care Outcomes Assessment Program (SCOAP) is the future of surgical quality improvement. It is a physician-led, voluntary collaborative creating an aviation-like surveillance and response system for surgical quality.

Obstetrical
CARE OUTCOMES ASSESSMENT PROGRAM (OBCOAP)

The Obstetrical Care Outcomes Assessment Program (OB COAP) is built on this same foundation of success and focuses on the decisions made during labor and delivery. Following its pilot program in 2010, OB COAP has steadily grown and now has participants representing approximately 1/3 of the births in Washington State as well as hospitals outside of Washington.

Smooth
TRANSITIONS

Smooth Transitions is a statewide quality improvement effort that began in 2009 under the auspices of the Washington State Perinatal Collaborative. The project seeks to improve maternal and infant outcomes through the development and implementation of clear transfer processes, with continued evaluation through education, data review, and relationship-building.

PATIENT SAFETY COALITION

The Washington Patient Safety Coalition brings together a diverse group of participants to build relationships and focus on and achieve common patient safety goals. As national attention has turned toward medical errors, patient safety and best practices is here to help make sure health care is safe for you and loved ones.
