Protected Reviews

General Information

    • Smooth Transitions now has state-sanctioned protection for two different confidential case review processes: IPCR and EPCR
    • The purpose of protected case review is for multiple disciplines to come together to review a transfer and identify opportunities to improve the process and outcome for our shared patients
    • Both review processes are intended to be educative, not punitive, and the learnings will be used to inform and generate best practices among participants

IPCR Details

    • Internal Protected Case Review (IPCR) is for community and hospital providers (and EMS, if involved) to come together for a protected case review of a specific community-to-hospital transfer
    • This type of review addresses the need for more discussion and/or clarity around a specific clinical case and/or complex systems issues.
    • Only participants in Smooth Transitions can use this process.
    • The findings/recommendations from the review will only be shared with involved parties.

  1. Request for IPCR is submitted to the Smooth Transitions Program Manager (STPM).
  2. The STPM emails out the IPCR Introduction letter, the ST Participation Agreement, and the IPCR Confidentiality Statement to involved parties*.
  3. All participants must sign the ST Participation Agreement and IPCR Confidentiality Statement to proceed.
  4. The STPM coordinates with participants to schedule a date for IPCR and clarifies the style of the review (in-person, virtual, or hybrid).
  5. The primary practitioners prepare their care summary and self-analysis to present at the IPCR.
  6. Following the IPCR, the STPM sends out a final IPCR letter of participation to the involved parties.

*If a group has previously participated in an IPCR together and all appropriate documents are complete, a subsequent IPCR may be requested; the STPM should be contacted for coordination and scheduling.
