History of TRAX: 

• December 2022, the Washington Healthcare Forum (Forum) Board composed of health delivery systems, health plans, Associations, and others, and senior leadership from the Washington State Department of Health (DOH) agreed to develop a joint plan for improving public health chronic disease surveillance in Washington State while creating an innovative solution for data sharing.
• December 2023 the Forum Board and DOH Executive Team members agreed to support implementation planning.
• Date for technical build
• Date for MultiCare Pilot
• April 2025 the Foundation for Health Care Quality was contracted to be a third party convener for TRAX Governance
