ABOUT THE SCORE CARD BANK

The Score Card Bank makes it easier than ever to find, download, and submit score cards for Bree Collaborative reports. Organizations can access score cards for all active Bree topics in one convenient location. The purpose of these score cards is to collect and share standardized information on the extent to which organizational policies, contracting, programs, and care processes are concordant with the guidelines.

If your organization previously received an award for a topic that is currently paused for revisions, you are welcome to reapply once the updated score card is available. Reapplying allows you to reaffirm your commitment to best practices or establish a new baseline as you update your programs and policies to align with the latest recommendations.

Please note that score cards are not being accepted for reports currently marked as Paused for Revisions. New score cards are being developed for these reports and will be available once revisions are complete.

Using score cards for baseline data

Score cards are a valuable tool for organizations beginning to implement Bree Collaborative recommendations. Even if implementation is just getting started, organizations are encouraged to submit baseline data to the Bree Collaborative. This information can help measure progress over time and identify opportunities for improvement.

Awards

Organizations and providers that submit score cards may be eligible for Bree Collaborative TRAILBLAZER and PATHFINDER awards, recognizing their commitment to implementing evidence-based best practices.

Data Use

De-identified data may be used to populate dashboards and report on process measures across participating organizations.

Identifiable data may be used to support Bree Collaborative reporting on the adoption of our recommendations. At this time, individual organization or provider scores are not publicly shared.

Follow-Up

The Bree Collaborative may contact organizations to learn more about innovative implementation strategies, successful approaches, or adaptations of recommendations to meet local needs. These examples may help inform future guidance and promote shared learning across participating organizations.

How to Submit Score Cards

Excel versions:

  • Download the appropriate score card(s). More than one may apply to your organization, for example, score cards for health systems and clinicians.
  • Fill out score cards using the self-scoring scale provided.
  • Attach any relevant documentation.
  • Email the score card and attachments to Karie Nicholas at knicholas@qualityhealth.org or to bree@qualityhealth.org

GIS versions:

  • Open and fill out the on-line version. You are able to save and continue your work at a later date. Please note, if your score card doesn’t let you save, please contact us.
  • Attach any relevant documentation in the file attachment field.
  • Hit submit at the bottom of the form

Share Your Own Evaluation

Other Ways to Contribute to Bree Collaborative Evaluations

Submit a survey question or full survey to the Evaluation Survey Question Bank

The Bree has developed a survey bank and a survey question bank, modeled after the CDC Question Bank, to reduce the burden of evaluation work and improve the validity of evaluation findings. The banks allow multiple organizations to use the same surveys or questions when they perform PDSAs or evaluate programs or implementation projects.

Organizations can submit “homegrown” surveys to the survey bank. Individual questions can be submitted to the question bank for shared use. The Bree can also facilitate data sharing to create a large sample size for survey question validation. Our Question Bank can be found HERE  and our Survey Bank can be found HERE.

Submit a measurement definition to the Measurement Bank

Similar to the survey question bank, the measurement bank allows organizations to submit definitions for their “homegrown” measures for use by others. The aim of the question bank is to help organizations create measure alignment where it currently doesn’t exist. Our Measurement Bank can be found HERE

Contact us about developing a Case study

Each year the Bree chooses a topic area to develop new case studies that can illustrate impact and demonstrate the “how to” of implementation. Your project doesn’t need to be on one of our defined topics in order to submit a suggestion, we are always open to new and interesting ways that organizations have used our guidelines. You can read our current case studies or submit ideas for your own HERE.
